New Adult Passport: Age 16 and Older Download
Select this process to view instructions to obtain a passport for applicants age 16 and older.
This process applies to persons who have never been issued a US passport; or have an Adult passport that was issued more than 15 years ago, or have a passport that was issued before the applicant’s 16th birthday.
Minor Passport: Under Age 16 Download

Passport Renewal Download
Select this process to view instructions to obtain a passport renewal
This process applies to persons who have a passport that was issued after the age of 16 and will expire within one year;
OR have a current passport that was issued more than one year ago and after the age of 16, and have experienced a legal Name Change;
OR have an expired passport that was issued after the age of 16 and the issue date is less than 15 years ago.

Second Valid Passport Download
Select this process to view instructions to obtain a Second Valid Passport with a 2 year limited validity.
This process applies to applicants whom are flight crewmembers and require a second valid passport to avoid the difficulty of obtaining visas for frequent international trips;
OR travel to destinations in the Middle East which may create difficulties with safety and security.

Second Valid Passport Renewal Download
Select this process to view instructions to obtain a new Second Valid Passport with a 2 year limited validity.
This process applies to applicants whom have a 2 year passport (second valid passport) and it has expired or is expiring within the next 6 months.
Additional Passport Pages Download
Select this process to view instructions to obtain additional visa pages in your passport for visas and stamps.
Additional Pages can be added to a US Passport a maximum of three times, however acceptance of each request is determined by the Passport Adjudicator’s discretion. Adjudicators can reject an Additional Pages request in preference of a Passport Renewal.
Lost or Stolen Passport Download

Mutilated Passport Download

Passport Re-Application Download

This process applies to persons whose passport has an issue date of less than 1 year ago; AND whose name has changed since this passport was issued, or whose identifying information was printed incorrectly in the passport, or whose passport was limited for one year and issued abroad.
