New Adult Passport: Age 16 and Older



This process is for applicants who have never been issued a US passport, or have an adult passport that was issued more than 15 years ago, or have a passport that was issued before the applicant’s 16th birthday.


Minor Passport: Under Age 16


This process is for applicants who are under the age of 16 and have never been issued a US passport, or are under the age of 16 and have been previously issued a US passport.


Passport Renewal


This process is for applicants who have a passport that was issued after the age of 16 and will expire within one year, or have a current passport that was issued more than one year ago and after the age of 16 and have experienced a legal name change, or have an expired passport that was issued after the age of 16 and the issue date is less than 15 years ago.


Second Valid Passport


This process is generally for applicants who are flight crewmembers or frequent business travelers and require a second valid passport to assist with the difficulty of obtaining visas for frequent international trips, or who travel to destinations in the Middle East which may create difficulties with safety and security.


Additional Passport Pages


This process is for applicants who have a passport that will expire in more than one year and has very few blank Visa pages remaining in their passport book.  Additional Pages can be added to a US Passport a maximum of three times, however acceptance of each request is determined by the Passport Adjudicator’s discretion. Adjudicators can reject an Additional Pages request in preference of a Passport Renewal.


Lost or Stolen Passport


This process is for applicants who have been previously issued a passport booklet and that book has been lost or stolen.


Mutilated Passport


This process applies to applicants who have been previously issued a passport booklet and that book has been damaged or is in very poor condition.


Passport Re-Application


This process is for applicants whose passport was issued less than one year ago and one of the following scenarios have occurred:  their name has changed since this passport was issued, their identifying information was printed incorrectly on the passport, or the passport was issued with limited validity.
